Type 2 Diabetes Prevention

AI/AN have a greater chance of having diabetes than any other US racial group.
Strategy 3 aims to support type 2 diabetes prevention amongst American Indians and Alaska Natives by implementing evidence-informed and culturally adaptive community clinical linkages. The primary objective of this strategy is to increase enrollment of community members at high risk for diabetes in CDC-recognized type 2 diabetes prevention programs.
Activities:
- Increasing awareness of prediabetes among tribal members and health care providers/ health professionals (e.g., community wide events, educational campaigns, health-care provider/health professionals training)
- Supporting prediabetes screening, testing, and referrals to CDC-recognized type 2 diabetes prevention programs by health care teams, community partners, health para-professionals
Outcomes:
- Increased number of community members at high risk for diabetes enrolled in CDC-recognized type 2 diabetes prevention programs offered in AI/AN communities.
